Are you legally authorized to work in the United States?
Do you know of any reason why you cannot perform the essential functions of a firefighter with or without reasonable accommodations?
Have you ever been convicted of, found guilty of, or pled guilty to any criminal offense?

(An affirmative answer will not automatically disqualify you from being considered as a candidate for employment.)
